Dear Golfer,
The fourth annual golf outing for Huntington’s Disease Society of America – Illinois Chapter will be held at Antioch Golf Course on August 7th. Registration will begin at 10:00am with the first tee off at 11:00am. Thanks to people like you, last year was very successful. Again this year we are including dinner at the end of the outing. Dinner will be held at the clubhouse at approximately 6:00pm. The dinner will be buffet style with excellent food to choose from. If you have someone who does not golf but would like to join us for dinner, that option is also available. We hope you will be able to attend again this year and stay for dinner.
GET A TWO OR FOURSOME TOGETHER AND HAVE A GREAT DAY OF FUN!
Included in this newsletter is a registration form. Please complete the form and return it with your check as soon as possible so that tee times can be assigned. If you need additional copies of the registration form, please feel free to make copies.
